I work with patients who have a need in post-settings. I must make decisions for patients in acute settings based on clinical significance. I must use evidence-based information to help me make the best decision for their post-acute setting and discharge plans. There are a variety of settings for post- acute care such as nursing homes, rehabilitation facilities, and home health agencies (Bryant et al, 2004). Measuring outcomes of evidence- based practice helps implement changes that are designed to improve patient outcomes and care process (Bryant et al, 2004).
